Before your Appointment

Here’s a quick and easy checklist to help you prepare for your appointment!

What to Bring
(New Clients/Patients)

Yourself & Your Pet!

Be sure to bring both yourself and your furry friend to the appointment! Please keep them leashed and/or safely secured in a carrier. If someone else will be bringing your pet on your behalf, just give us a quick call to let us know. This other person will need to take financial responsibility when bringing in your pet so their name will need to be added to the registration form as well.

Previous Medical Records

Don’t forget to bring along any previous vet records, vaccination history, or other important medical documents for your pet. Want to make it even easier? A quick call to your previous vet to have them email us your records works like a charm!

Questions/Comments/Concerns

A few days before your visit, start jotting down any questions, concerns, or observations you’d like to share with the veterinarian (bonus points for a joke to keep things light!). Consider noting changes in diet, behavior, or bowel movements.

If these concerns are tricky to explain, a photo or video can be a great way to provide helpful details!

Medications

Be sure to bring a list of any medications your pet is taking—or better yet, bring the bottles along! That way, we won’t you to remember how to pronounce those tricky names.

Stool Sample *

Stool samples are used for fecal tests, where we check for parasites that might be present in your pet's stool. Fresh is best (within 24 hours), and you can bring it in a small plastic bag for convenience.
